Referral Coordinator

Department: Special Projects, Innovation
Location: Danvers, MA

Schedule: Monday - Friday 8 am to 4 pm

Target Salary: $24.51 - $28.84 per hour

Please note that the pay range provided is a good faith estimate for the position at the time of posting. Actual compensation will be dependent on multiple factors, including but not limited to, the individual's experience, education, and internal equity.

Job Summary

The Referral Coordinator is a key role within Northeast Arc serving as the centralized point of contact for referrals and ensuring a responsive, organized, and person-centered intake process. In this position, you will inform and guide individuals and families through available services to encourage a smooth intake process from the initial inquiry through program placement.

The Referral Coordinator is responsible for collaborating with internal program teams, supporting individuals and families with required documentation, and maintaining accurate records in the electronic medical record (EMR) system. This role also steps in to support existing staff when referrals do not meet program eligibility or when additional services are needed. Aligned with Northeast Arc's mission of empowering individuals with disabilities to live, work, and thrive in their communities, the Referral Coordinator plays a critical role in optimizing referral workflows, tracking data, and enhancing the overall experience for individuals, families, and stakeholders.

Responsibilities

  • Serve as the primary point of contact for internal and external referrals, ensuring a responsive, organized, and person-centered intake process
  • Coordinate referrals from initial inquiry through program placement by collaborating with internal program teams
  • Support individuals and families with required documentation throughout the referral process
  • Maintain accurate and up-to-date records in the electronic medical record (EMR) system
  • Support existing staff when direct referrals to programs do not meet eligibility or require additional services
  • Optimize referral workflows to improve efficiency and enhance the overall experience for individuals, families, and stakeholders
  • Track referral data and identify trends to support continuous quality improvement

Qualifications

  • Bachelor's degree in administration or related field preferred
  • A minimum of two years' work experience; experience in developing and implementing new systems and/or project management preferred
  • Excellent time management, multi-tasking and organizational skills, with the ability to simultaneously manage and execute several projects or tasks
  • Strong written and verbal communication skills
  • Highly detail-oriented with the capacity to work independently and prioritize workload
  • Advanced computer literacy skills, including Microsoft Outlook, Excel, Word, and PowerPoint
  • Ability to manage confidential issues and materials discreetly
  • Requires someone to sit for long periods of time, talk on the phone, and enter information into the computer for at least 70% of their day; infrequent light physical effort required
  • Must successfully complete and pass all required background checks as a condition of employment
