Program Coordinator- Journeys

Department: Day Program
Location: Danvers, MA

Job Title: Program Coordinator- Journeys

Job Summary: The Program Coordinator is responsible for supervising, guiding, and training program case managers and direct support staff in the community-based supports program. This position plays a pivotal role in creating and implementing individualized opportunities and skill development programs designed to enhance the independence, self-sufficiency, and community integration of the individuals served by the Northeast Arc. The Coordinator ensures that all program activities and staff practices adhere to agency policies and funding source regulations.

Hours: Monday through Friday 8 am -4 pm

Major Responsibilities:

Leadership & Supervision

  • Supervise and mentor Program Case Managers, providing guidance, training, and performance evaluation to ensure daily responsibilities are met.
  • Assist with managing staff ratios across the program and maintain effective communication with other coordinators and the Program Director regarding staffing and operational changes.

Client Support & Planning

  • Oversee assigned caseloads and maintain regular, professional communication with each participant’s support team (families, residential providers, funding agencies).
  • Develop individualized goals and meaningful, cost-effective activity plans based on participants’ interests and visions for skill development.
  • Ensure participant progress toward established goals and provide direct support and coaching during volunteer placements and community integration activities.
  • Serve as a professional role model and make sound clinical decisions and interventions, escalating issues to supervisors when necessary.

Operations & Compliance

  • Complete all required assessments and necessary paperwork for annual support meetings and HCSIS submissions within mandated timelines.
  • Maintain all required documentation within specified timeframes, including daily data, monthly/quarterly progress notes, interest inventories, assessments, and confidential record updates.
  • Plan daily schedules, ensure all protocol specific to each person served is followed, and assist with program transportation using personal or agency vehicles.
  • Attend and complete all assigned mandatory training sessions.
  • Provide direct physical and personal care support as needed, ensuring the health, safety, and dignity of all individuals served by the program.
  • Communicate effectively and professionally in all written reports, documentation, and oral interactions with staff, families, and external partners.
  • Demonstrate a thorough understanding and consistent application of all Northeast Arc policies, Day Service Division standards, and external funding regulations.
  • Strictly adhere to all HIPAA laws and security guidelines to ensure the confidentiality and privacy of all participant information.

Education and Experience: High School Diploma or equivalent and one to two years’ experience working with intellectually and/or developmentally disabled adults.

Skills, Knowledge, and Abilities

  • Communication: Excellent written, verbal, and interpersonal communication skills required, including the ability to communicate effectively in English and/or American Sign Language (ASL).
  • Organization: Strong organizational abilities with excellent attention to detail, time management, and the capacity to prioritize tasks effectively.
  • Adaptability: Must be a flexible team player who can adapt quickly to changing schedules, priorities, and client needs.
  • Technology: Proficient working knowledge of Microsoft Office products (Word, Excel, Outlook) and associated applications.
  • Requirements: Must possess a valid driver’s license and demonstrate the ability to operate agency or personal vehicles safely for transportation needs.

Physical Demands: Must be able to stand/walk up to one hour uninterrupted.

Work Environment: Environment consists of program areas, volunteer sites, vehicles and various locations out in the community.
